Looking at the photo you have show, I think you may require another neck lift combined with a lower facelift to improve the appearance of the ripples under your chin.

A combination approach would give the most improvement. From your picture it looks like you have a return of the skin laxity under your chin which probably prompted your neck lift in the first place. I would return to your plastic surgeon for an evaluation, and request a secondary tightening of your skin. I wish you the best!

Most likely, the issue in the photo represents a stitch abscess and you should bring this to the attention of your Surgeon. This is not likely to be a major issue, but requires more than Neosporin.
