An examination is needed to be certain, but you probably could achieve improvement with a revision. If you have any donor fat, that would be helpful to give more volume in the lower medial areas of your breast.

I recommend a circumareola lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers can be performed if additional volume is desired.
