I had my eye bags removed about 20 years ago, but now am noticing an emergence of bags. Do you accumulate more fat in that area with age? I'm trying to decide if I should wait or seek treatment? One surgeon says I have enough fat to reposition and another surgeon says I don't have enough fat and just wants to remove what's there, but I'm afraid of having a hollow look. I don't like the idea of filling with fat as I'm afraid of ocular occlusion,. Please help!

The picture show negative vector and taking the fat out through transconjuctival incision does not help. You will need tear trough implant and subcillary( skin) incision.
Without seeing pictures, we can’t really begin to make an assessment.

The lower parts of your eyes demonstrate scleral show. I would advise against surgery to address this. The results can be extremely unpredictable and are likely to leave you dissatisifed.
