I have had lower eyelid surgery 2 times in 2 years, but I still have bags under my eyes. It seems like they are getting worse. My plastic surgeon says he reconfirmed during the second eye surgery there was no fat. The swelling is worse some days than others, so I wonder if it’s my lymphatic system that is. It working properly.

Answer: Without an actual physical examination, it is impossible to know the basis for these issues.
Without seeing pictures, we can’t really begin to make an assessment.

The lower parts of your eyes demonstrate scleral show. I would advise against surgery to address this. The results can be extremely unpredictable and are likely to leave you dissatisifed.
It appears your eyes are deeply set and your eyebrows are ptotic and too low. You would benefit from a browlift and possibly an upper blepharoplasty. You may also benefit from fat transposition into the hollow areas. This can be done by repeat lower blepharoplasty or fat injections.
