I’m a 27 year old female who lost 145lb. I have a lot of loose skin on my abdomen but it retracted pretty well for how much weight I have lost. I’m in my 4th year of veterinary school and currently on rotations, so I have a limited amount of time (4 weeks) for recovery before returning back to clinics. I was contemplating a TT with no muscle repair as I think it would cut the healing time, but am afraid of looking boxy. I had a consultation with one surgeon who said that I probably could get by without muscle repair, but I’ve seen some really bad TTs and am scared.

First of all, I do not plan a surgery "that you probably could get by without muscle repair" and hope others don't either. As a board certified plastic surgeon with years of experience in body sculpting techniques, I do what will give the best possible results for my patients based on their anatomy. Yes, you can do less, but then you are setting yourself up for another surgery. A secondary surgery might not turn out as good. I would recommend a full abdominoplasty with muscle plication, low pubic bone level incision and lifting the sagging pubic tissues. I do these surgeries in our office surgical suite under deep intravenous sedation and not general anesthesia. Initial recovery is 7-10 days, but in one month you can return back to your clinical duties. Good luck.

Answer: Bellesoma Method and tummy tuck I would recommend beginning with your breasts as your breasts are hanging quite low. The reason I begin with the breasts is because the breast lift will pull up the upper abdominal skin and will eliminate the need for a fleur de lis. The breast lift technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. At the same time, you can undergo a tummy tuck.
