If you got your injections done by a reputable clinic i don't think that you have anything to worry about. It is very common to discard the box and syringe after injections . Each treatment is documented so if you have any concerns you can follow up with your injector .   

Please go back to your injector and let them see. It is impossible to tell from the photo. If the bumps are from juvederm they should go away with massage and can also be removed with hyaluronidase. 

I am assuming that you had the Juvéderm placed in your lips. If so, then not, I doubt that it is the cause of your perioral dermatitis. I would suggest that you let your injector know what is going on and he or she can contact Allergan.
