Breast implant exchange risk for BIA-ALCL? Would you recommend capsulectomy or capsulotomy?

I had a breast augmentation 4 years ago with textured allergan implants which were recalled due to the BIA-ALCL risk. I got 475g anatomical implants placed over the muscle. To this day I have not had any issues whatsoever, but I would like to exchange my breast implants for aesthetic reasons and due to my worry of the cancer. So my question is: would you recommend capsulectomy (en bloc?), capsulotomy for implant exchange? Thank you!

Was the shape of you breast different in the immediate postop time? Do you feel the lower pole of the affected side is now longer? What plane are the implants in (over or under the muscle?)It is very normal for the implants to settle and continue this process for several months after the surgery...
It is possible to change the size of your implants to something smaller. When decreasing size it is important to modify the pocket to match the size of the new (smaller) implnant. Often some form of internal bra is helpful to provide support when creating a change in pocket size.
It is very normal for the 2 implants to behave differently. This is not something to worry about. In your case the implant may move to the side because of the lateral extension to the pocket. Meaning that when creating the pocket for the implant at the time of your surgery the implant on the...
