Providing that first of three photos is before surgery I would say that you have some improvement. The issue with far grafts in that area is that about 30% of injected volume will take after 6-8 months. In some patients additional fat grafting may be needed to provide optimal result. The junction between the midface and lower eyelids is highly complex, and there are different approaches to how it should be treated.
Whenever we have a complex situation, it’s more likely that providers will have different recommendations and approaches.
In my experience fat grafting in this area is imprecise, unpredictable and potentially unforgiving if patients don’t like the outcome.
In my opinion filler is the gold standard for midface volume restoration. Your pictures are a bit limited, but based on those, it looks like you may have some malar edema. This is a condition that causes puffiness in the area and we really don’t know why some people get it nor that we have a good treatment for it.
I have heard some people feel it gets worse with the use of fillers but I have not seen this personally on any of my patients.
Midface volume restoration can be treated with fillers, fat grafting, implants, or a midface lift.
As I mentioned this anatomic region is a bit complex.
