Question

I had intramuscular butt implant surgery October 2018 (500cc) with no complications. I had a second butt implant surgery April 2018 (700cc) with no complications. At the same time as my 2nd implant surgery I also had liposuction on my thighs below my butt. With the second implant surgery I was hoping to make the lower loose flap tighter, and possibly take up some of the skin below my butt, but it made it worse. Will a lower butt tuck make my butt and upper thigh look like it should?

Provided that the implant size and shape is optimized, at this point, a lower buttock tuck may indeed be your final and best option.A buttock tuck is kind of like a tummy tuck for the buttock in that it removes loose sagging skin from not only the lower buttock but also the back of the thigh to tighten both areas and create a more defined buttock crease. In doing so it also aesthetically shortens the buttock cheek. It is ideal for those patients that have had significant weight loss or fluctuations, changes from fillers injected, multiple pregnancies, or who’s derrieres have succumb to the dreaded effects of gravity and aging. It is also effective for those whom feel their buttock cheek is naturally too long and just want it proportionately shortened.

Like a well done tummy tuck, the 3-dimensional planning and surgical technique is key. It is best done under general anesthesia while the patient is in the prone (i.e. face down) position. The loose and/or elongated lower buttock and upper thigh skin, subcutaneous fat, and connective tissue are all excised. The fascial edges are brought together with deep  stitches secured to the gluteus muscles effectively lifting the back of the thighs at the same time. The resultant scar is strategically placed within the junction of where the buttock cheek meets the thighs.  In many patients this scar is well worth the improved buttock/thigh contour.

Many patients travel a great distance for the inferior buttock lift/posterior thigh lift. An incision is made in the natural gluteal fold (the line under each buttock) and several inches (7 cm) or more from the inferior buttocks, and another several inches (7 cm) from the posterior thigh can be removed. This procedure can produce dramatic improvement in buttock shape and smoothness as well as skin laxity and the appearance of cellulite. I will need to examine you to determine if the procedures will work for you.
