I'm transgender. Went from 264lbs to 153lbs, had a lot of excess skin & totally deflated butt, then had a tummy tuck. Was told at 153lbs I had no fat for BBL, so went to 195lbs & then had a BBL with 1600cc per side (Dec 2018), but my butt has no projection (5 months post-op, 195lbs). Second BBL might still not give me projection, maybe I still have excess skin on my butt. Should I get a traditional butt lift instead of second BBL next? Butt implants scare me; complications, long recovery.
Answer: Buttock implants for deflated butt after massive weight loss On a patient like yourself, the only way to predictably, reliably, and permanently restore ample volume and thus projection is with buttock implants. More fat grafting will likely produce similar (disappointing) results. A traditional buttock lift will further flatten the buttock not add projection. Bottom line, buttock implants can be and are performed with a high level of safety and efficacy when done by an experienced reputable specialist. Very few plastic surgeons specialize in buttock implants but all know well how to perform BBL, thus BBL is so highly promoted and, frankly, overpromised. As far as risks and complications, BBL has a much much higher risk of serious complications such as death compared to buttock implants. Glad to help.

Answer: Lost a lot of weight, butt has no projection after BBL. Should I get a traditional butt lift next? (photo) Based upon the photos, removal of excess skin would probably generate the smoothest, best result.Kenneth Hughes, MD, ABPS Board Certified Plastic SurgeonLos Angeles, CA

Answer: Procedure of choice after BBL? Hi and welcome to our forum! There is perhaps no single answer here, but from your photos, I see much better definition of the buttocks since your procedure, but there is significant skin redundancy. Under these circumstances, I would recommend a beltectomy to lift the buttock and eliminate lax skin of the lateral thigh, hip, and buttock.Seek a consultation from a board certified plastic surgeon.Best wishes...

Answer: Butt lift Hello and thank you for your question. Congratulations on your weight loss. I believe what need is a buttock lift with auto augmentation, that is using your own tissue as an implant ( not bbl ). Would also suggest going back down to that 153lb again. If you get a traditional lift your buttocks will be flattened even more that’s why I suggest the augmentation at the same time.Peter Fisher M.DPost massive weight loss body contouring specialist
