Is a lower butt lift (tuck) the best option for my elongated flat butt? Your opinions please (Photo)

Question

After visiting a plastic surgeon I respect, recommended lipo of outer inner thigh waist flanks for contour. Than cut raise and create a new crease on my lower butt to shorten my butt. Tighten and lift skin under my butt. Then replace the small amount of fat to higher section of butt (make a little difference) I do not want butt implant & not looking for big butt just a round and shaped butt & back of thighs please share your thoughts. ??

You have a very elongated buttocks because of your genetics and also because of your buttocks Being flaccid. So you will be a great candidate for  Buttocks Lift using a low incision to prevent elongating the same way. In combination with it I would recommend to do some liposuction to lower back to create a better definition in between the back and buttocks and create a more curvaceous area. And minor lipo to outer thigh. And use that small amount of fat to transfer to your buttocks which will NOT be big because your do not have fat enough, but definitely nicer shape. Just adding volume will NOT lift your buttocks  Be encouraged. Dr. Cárdenas.

Although you may not want buttock implants, the only way you will get anywhere near your wish pics is with buttock implants to restore volume, fullness, and that desired rounded profile curve. Then possibly six months later you may further benefit from adding a lower buttock tuck to better define the lower buttock/thigh junction.  Just performing liposuction plus/minus a lower buttock tuck will leave you with an even flatter buttock. You simply need much more volume than any fat transfer will provide on your very lean body. Glad to be of help.

After a Brazilian Butt Lift, usually the butt is more voluptuous and fuller and lifted and the area is rejuvenated. There are other combination of procedures that can also work on the love handles, the lower back and the thighs to affect shaping. It also depends on what specific procedures you had done as well as how your recovery is going. I routinely tell people to take a few days to a week off work and to hide out but some people are very nervous especially celebrities and they might hide out for two weeks. As with any scar on the human body, initially scars are red, hard and raised and it typically takes six months to a year for them to become a soft thin white line. Additionally with recovery, there're many things we do to expedite healing such as nerve blocks during surgery, oral supplements, skin care and scar therapy recommendations, lymphatic massage sessions that are included in our pricing.
I strongly feel that it's very important to have an honest conversation with your surgeon about your particular situation, lifestyle, work environment and specifically which procedures you're having done so that you both have the most honest answer about your recovery and are realistic with expectations and results. I would recommend that you see a board-certified plastic surgeon in your area that is a member of both ASPS & ASAPS and performs a lot of butt lifts.

To enhance the buttock with contour, frequently fat grafting is performed.  One needs enough fat for grafting to enhance shape and “lift’ the buttock- abdomen, waist and thigh liposuction is often used for BBL.  If there is skin laxity and sagging, a buttock lift can be approached with an incision below the buttock in the gluteal crease or above the buttock to lift or a combination of both.

Than you for your question and I see your concerns.  It appears as though you are pretty slim.  In pictures it's difficult to assess how much fat you have available to transfer, but that would be your best option to achieve something similar to the wish pics you have included.   The additional volume added to your buttocks will fill it out somewhat and improve your projection, minimizing the drooping somewhat.  The Lower Buttocks Tuck that you're referring to is what I call a Buttocks Wedge Excision.   This would be an option to tighten your skin in this area and shorten your buttocks.  It will also make it somewhat rounder in the lower portion where it is currently flat.  But, I believe Fat transfer to your Butt would give you by far the best result.   You might need to gain a few pounds to to have enough fat for this though.

Depending upon the extent of  buttock ptosis, treatments may vary. Minimal ptosis can be corrected with buttock augmentation, either with fat injection or buttock implant. Moderate ptosis is treated with crescent excision and deepithelialized flap closure locally at the inferior buttock crease. Severe ptosis requires a beltectomy to lift the buttock from above but creates a significant scar at the lower back. 
Your plastic surgeon appears to have given you sound advice.

Hello and thank you for your excellent question. Based on your description, yes I think that your plan is entirely reasonable. I have found that many patients can get GOOD results with liposuction alone, and EXCELLENT results with IDEAL Liposuction! IDEAL Lipo is a proprietary technique that we have been developing for the past 12 years. First off, these procedures are typically performed in the office, with only local anesthesia (although patients can request deeper sedation, if they so desire). We typically use Smart / Laser Liposuction or VASER (ultrasound) to reduce dense, fibrous fat, and stimulate collagen formation; Radiofrequency-based skin tightening (like Body Tite, or Thermi) to maximize skin contraction; and Ultrasound, RF, or subcision (Z Wave, Venus, Cellfina) to treat cellulite. Patients go home the day of surgery; typically do not use any narcotics postop for pain control, and are back to light exercise and work 48 hours after their procedures. In my practice, we specifically evaluate patients for these additional treatments, and select the appropriate treatment based on their skin quality, amount of fat, previous treatment(s), and surgical goals. It is best to answer your question during a face-to-face evaluation, when you can discuss your goals and expectations for the procedure, and you can have a physical examination to evaluate for that procedure. I recommend that you find a board-certified, or board-eligible plastic surgeon with whom you are comfortable. Be sure to have all of your questions answered during a face-to-face meeting with your surgeon, and review before and after pictures of similar patients whom have had this procedure.
