I’m looking for the attached change. Does this procedure require a rib graft and can it be done with fillers? Is it too subtle to recommend undergoing surgery if the patient is ok with the way it looks now - is the risk of a worse outcome too high?

Answer: If the patient is happy with their current nose and only wants a subtle change There's no reason to undergo a Rhinoplasty procedure if you only want a subtle change. From the photographs presented, it appears that you just need A dorsal Hump reduction and a release of the depressor septi ligament which dynamically pulls the tip downwards when smiling.
