I am wondering if what I have is gyno/glandular or if I need to just lose some weight. I have always had “puffy” looking nipples but not sure if surgery would fix it. 29 years old 6’2” 195 lbs. also, is it better to have the surgery with some extra fat or when lean?

Gynecosmastia surgery can be done with an incision made around the areola to remove the excess tissue and tighten the skin. Losing weight will not result in loss of the tissue. Answer: Gynecomastia or fat? Gynecomastia is enlargement of the breast tissue in a male. Based on your photo and your description it appears you have some gynecomastia. It is treatable with surgery. The reason to do surgery is because you don't like the appearance, otherwise it is safe to leave it there. Surgery is best carried out when a person is at a healthy weight, there is no need to adjust your weight for surgery.
