I had Gyno surgery in 2011 when I was 15 or 16. I am now 27 and I feel like my chest still looks weird under the nipple. I am overweight right now, but working out with a trainer. It is hard to activate my chest when weight lifting or benching. Also there feels like my chest is sunken in under the nipple. Will my chest look better as I do more chest weights and lose fat or does it need to be corrected through surgery?
Answer: Ten years after Gynecomastia, residual deformity Thank-you for your question. Based on your photographs , you still seem to have some Gynecomastia and also som loose skin. You will need to see your Board certified Plastic surgeon to examine you and possibly do some revisional surgery for contouring your chest. Working out will help but my not correct this problem. All the best!

Answer: Gynecomastia surgery Hi When ı examined your photos, ı can see that you have too much skin excess. So ı do not believe that tis will go away by exercise or diet. You need skin, gland tissue and fat tissue removal at the same time. The operation lasts 1.5 hour. You stay one night in hospital. You must give a break to sports for one month.

Answer: How to assess gynecomastia and surgical outcomes For the Male chest there are three tissue variables that need to be individually assessed to get an understanding of what the problem is. Likewise when it comes to treatment options all three of those tissues need to be addressed if indicated. The three tissue layers involved are skin laxity, excess subcutaneous fat and glandular breast tissue. It sounds like you had a glandular breast tissue removed so that part may be well-controlled at this point. You are somewhat obese and have excess subcutaneous fat. Your chest also shows clear skin laxity. To get an appropriate result regular gynecomastia surgery will be very limited. Skin laxity contribute significantly to contour in this part of the body. One way to differentiate how much skin laxity contributes to the contour is to put the skin and pectoralis muscle under tension. Do that by taking ONE ARM AT A TIME and stretching the arm behind your neck like you’re trying to scratch your upper back. Have someone take a picture with your arm stretched behind your head or use the timer of your camera. you can also look in the mirror but it’s better to capture it on photographs. what does skin and pectoralis muscle under tension you will see the contour of the subcutaneous fat and glandular tissue clearly. As you slowly move your arm back down to a dependent position you will see the folders build up on your chest and this shows you exactly how much skin laxity is contributing to chest contour. To get a flat attractive chest you will need a chest lift. That procedure is not straightforward and not all plastic surgeons know how to do it or do it well. It does leave scars. Try to find all the major plastic surgeons in your community who seem to have a lot of experience with gynecomastia. Contact their offices and ask how many chest lift procedures they have done. Once you get an idea of who has experience with the procedure schedule multiple in person consultations. If you really want this done correctly you’re really need to hunt for the best provider. I recommend starting by having at least five in person consultations but in your case you may need to schedule more than that. During each consultation bring pictures of your old body with you to use for reference. Ask each provider to only show you before and after pictures of previous patients who are very similar body characteristics to your own. Remember there are three tissue variables that matter here. you should be seeing pictures of previous patients who first and foremost have the same amount of skin laxity. Secondly would be subcutaneous fat and lastly glandular tissue. because you’ve had surgery in the past glandular tissue has been reduced and most likely some subcutaneous fat was removed at that time as well. For that reason I believe skin laxity is the major component to your lack of results. The only way to manage skin laxity on the chest of a man is to do a chest lift. Ask providers to show you their entire collection of before and after pictures for this procedure. Again make sure the people in the before pictures look similar to what your chest looks like. You should insist on this. Ask providers to show you examples of excellent outcomes, average outcomes and outcomes that did not turn out as well as they had hoped. Ask each provider what the revision rate and revision policy is. For reference and experience provider should have no difficulty showing you at least 50 Set a before and after pictures of commonly performed procedures. Of commonly performed procedures surgeons shit in fact have hundreds or even thousands of before and after pictures. Gynecomastia is not the most commonly asked for a procedure but it is certainly not uncommon. Most people get subcutaneous fat liposuction with direct open glandular excision for typical gynecomastia surgery. I just left is typically not indicated. In your case because removing glandular tissue and most likely subcutaneous fat was insufficient it is pretty clear that a chest lift is most likely going to be needed. I don’t think you’re gonna find plastic surgeons who can show you 50 male chest lift cases but if you do you know you’re in the hands of someone who has a lot of experience. Regardless, I think you get the idea. The goal is to see as many before and after pictures as possible. Preferably those pictures should be representative of what your body looks like. Do this by having in person consultations avoiding virtual consultations whenever possible. In the end you may or may not desire to have the procedure. If you do please put in the sufficient work necessary to find the right provider. In the end there are only two variables that really matter when it comes to cosmetic surgery. Patient candidacy and skill of the surgeon. You are not a great candidate for traditional gynecomastia surgery. You are however a really good candidate for a chest lift. It is not a straightforward easy procedure but those who can do it well can give a really good results the way it usually comes with a significant scar. Be more selective than you think you need to be and have more consultations if you have any questions or doubts. The outcome will affect you for the rest of your life so for this one it’s worth putting in a significant effort to find the right provider. Good luck, Mats Hagstrom MD
