Bottom of breasts look “pulled down” and sit to the side, lack of upper pole - do I need to replace the implants? (Photos)

I had breast implants 2 years ago. 500cc n under the muscle low profile implants. Motiva. I was happy with them other than them looking too flat and having little upper pole fullness. My surgeon suggest low profile but I believe I should have had moderate. 1 year ago I had an arm lift/ armpit lift and a reverse tummy tuck. After this procedure, it’s changed the shape and look of my breasts. My surgeon has offered a revision by doing an inframammory lift. From researching, I am unsure if this will resolve the problem and if I need new implants including the lift. My biggest issue is the bottom of my breasts look “pulled down” and now sit to the side, and the lack of upper pole. I’d appreciate some expert advice and opinion from other surgeons. I have attached a before picture (when they were first done, 1 year post op. And a now picture)
