Breast Revision + Possible Lipo: Should I Address Back/Abdominal Fat During Surgery or Lose Weight First? (Photos)

Hi! I’m 28 and 5'5". Two years ago I had a breast augmentation with 240cc moderate-profile saline implants under the muscle. I started as a 32AA with very little breast tissue. The result looks “natural,” but I’ve never been fully happy - when I lie down I’m flat, the implants fall outward, and I have almost no cleavage without an underwire bra. My scars also healed poorly after an early infection, and I never developed a true inframammary fold. Since surgery my weight has increased from about 60kg to 80kg (muscle and fat). I recently consulted with a new surgeon I liked. She felt fat transfer alone wouldn’t give me the improvement I want and recommended an implant exchange about 100cc larger plus internal work to improve cleavage and implant position. She said she could add some abdominal fat but doesn’t recommend using back fat, and she may be able to improve my scars through the same incision even if my fold doesn’t fully form. My hesitation is liposuction. Most of the fat I’d want removed is in my abdomen and back, but abdominal lipo makes me nervous (I keep seeing “lipo belly” results). She reassured me that because I weight train and have good skin quality, I’m a good candidate. My question: Would I be missing an opportunity if I skip lipo during my revision - especially if it could improve both my breast result and overall shape? Or should I try to lose weight on my own and focus only on the implant revision? I prefer being curvier/having a butt, rather than lean...
