I'm not sure what I need to get my vision, maybe a lift, maybe some fat transfer? I'm against implants because I want my boobs to feel 100% natural. I like my size, but my shape feels very underdeveloped. I feel like my boobs are saggy and pointy. I thought that was just how small boobs look until I saw some that were rounded and not anything like mine. I realized I have teenager boobs instead of adult boobs, they started growing at 13 but I haven't seen any growth or shape changes since 19. I'm now 24 and starting to accept that I might need surgery since puberty failed me. I also want a nipple reduction. I'm horribly insecure of how my nipples are poking through everything. If I didn't have such unusual nipples I wouldn't even need a bra.

In my opinion, this is a shape and nipple issue, not a volume problem. Your description fits underdeveloped lower pole with mild sagging and prominent nipples.

The procedure I would recommend is a conservative breast lift (mastopexy) with nipple reduction. This reshapes your natural breast tissue, creates a rounder and more mature contour, improves nipple position, and keeps your breasts 100% natural in look and feel, no implants needed.

Fat transfer alone would not reliably correct the shape or nipple concerns you described.

In this situation, a nipple reduction/correction procedure alone can be performed without changing the breast volume or overall breast shape. The recovery period is generally quick.
However, I would like to emphasize an important point:In young patients who may plan to breastfeed in the future, we generally do not recommend isolated nipple reduction surgery, as surgical intervention to the nipple can, in rare cases, affect breastfeeding function later on.
For this reason, if you are considering future pregnancy and breastfeeding, **postponing this procedure** would be a safer approach. If there is no plan for breastfeeding, or if breastfeeding has already been completed, nipple correction surgery may be considered an appropriate option.
