I've had botox done on my forehead around 8 months ago. Since then my eye shape has changed in a way I can't really describe. It's like my eyes have sunk in or my eyelid is pushed out over my eye. From the side, my eyes have an almost flat appearance. It's almost like my upper eyelid is hanging loosely over the eye, and is creased and wrinkly. I look very tired, with dead eyes. I don't look like myself anymore. Could this be due to botox? And is it reversible on its own or will I need surgery?

I don't believe the vertical depressions on your forehead can be helped with Botox. This appears to be a mild thinning of the soft tissue in this area. I would recommend a hands on exam for an accurate diagnosis and possible solutions.
