Harvesting a rib from the chest wall takes about 60 minutes typically. The time consuming factor is what then needs to be done with the rib. We typically only use rib grafts in revision rhinoplasty cases or where there has been significant trauma to the nose. Typically opening the nose and preparing it for the rib graft takes many hours. It then takes time to carve the rib into the appropriate grafts and then to carefully stitch those into the correct positions. When you add all that up, the rib graft case takes from 3-8hrs, although I think about 5.5hrs would be an average amount of time.

4 hours is a long time but probably reasonable to do a complex rhinoplasty with a rib graft. Make sure you feel comfortable with your doctor and that you appreciate their results.
I can't conceive of someone taking seven hours to perform what you describe. If the surgeon is experienced with this type of surgery it should take three hours max.
The duration of ANY rhinoplasty (whether it be with or without cartilage grafts) is dependant on what is been done. When using cartilage from another site (ear, rib), one has to take into consideration the extra time in harvesting the cartilage. In rib, you have to take into account the rib harvest time, which can take 30-60 minutes, depending on the experience of the surgeon. The rest of the procedure (aside from the cartilage harvest) will depend on what needs to be done. How many grafts? Where the grafts are placed? How are they placed? The duration of procedure will also influenced by whether it is a primary or revision rhinoplasty. The need to provide strong cartilage support, potential soft tissue reconstruction, and the extra time in dissecting through a previously scarred environment in revision cases will obviously take a lot longer.Hope this information helps. How long does it take to do a rhinoplasty using rib cartilage? It normally takes 3 to 3 1/2 hours. You might ask a more important question which is why is there a need for rib cartilage. Having done rhinoplasty extensively for 35 years I almost never need rib cartilage. There is sufficient cartilage in the septum and both ears. You may want to have a second opinion with a very experienced rhinoplasty surgeon.

Hi, I have performed many Rhinoplasty Surgeries over the past 30 years. Local anesthetic that is injected into the nose to control bleeding, during Rhinoplasty, only lasts 2 hours. Any Rhinoplasty lasting longer than that will encounter significant bleeding leading to increased bruising, swelling and discofort post-op. I perform Closed Rhinopl;asty in about 1 hour and Open Rhinoplasty in 2 hours, even when using cartilage grafts. Rib grafts have a tendency to dissolve unevenly over 5 - 10 years and aren't a good graft choice for Rhinoplasty in my humble opinion.
