I can't conceive of someone taking seven hours to perform what you describe. If the surgeon is experienced with this type of surgery it should take three hours max.
The duration of ANY rhinoplasty (whether it be with or without cartilage grafts) is dependant on what is been done. When using cartilage from another site (ear, rib), one has to take into consideration the extra time in harvesting the cartilage. In rib, you have to take into account the rib harvest time, which can take 30-60 minutes, depending on the experience of the surgeon. The rest of the procedure (aside from the cartilage harvest) will depend on what needs to be done. How many grafts? Where the grafts are placed? How are they placed? The duration of procedure will also influenced by whether it is a primary or revision rhinoplasty. The need to provide strong cartilage support, potential soft tissue reconstruction, and the extra time in dissecting through a previously scarred environment in revision cases will obviously take a lot longer.Hope this information helps.
