You can improve your shape with liposuction. If you do not wish to have a larger buttocks, you don't have to transfer the fat there. It will look better proportioned if you make the waist smaller.
As you implied the buttock area can look more prominent with just doing liposuction around it and even in cases where fat grafting is performed more than half of the improvement is due to that surrounding liposuction. In your case the outer frame is A-shaped and the muscle soft tissue have a V-shape. Liposuction of the on each side of the lumbar spine, over the sacrum, above the hip crests and in the saddle bag areas will round the frame. If you want to round the inside of the frame you will need fat grafting to the upper outer quadrants of the buttocks and a small amount of grafting to the lower outer quadrants. Thank you for your photo. You appear to be overall thin but with localized deposits of fat situated on the flanks. This is extremely common and natural with aging. It is definitely possible for your butt to appear larger, more round, and even lifted with careful sculpting of your waist and flanks using liposuction. The eye will notice the proportion between your hips and cinched waist and you can get a curvier look without adding any fat back into your buttocks. You can change a waist line so much with liposuction and doing it under tum. local with the smaller canulas is the best way to try to get that smaller waistline.
