I'm a 32 A right now and my dream would be to wear C bra sizes but AT LEAST be a full B. Implants are out of the question. I want my breasts to be real but my hopes are being crushed after seeing and hearing that fat transfer to breast won't show a drastic result. I really want larger breasts but with my own fat. Can it be possible to go from an A cup to a C cup? Or at least a full B... I don't want to waste thousands for subtle results

Answer: 1 full cup size increase is usually the most realistic expectation Thank you for your question. The retained volume of autologous fat grafted for breast augmentation depends on multiple factors. There must be adequate volume of tissue within the breast to receive and revascularize the grafted fat. The greater the initial volume of the breast the greater the volume of grafted fat that may be accepted and retained. There must also be an adequate amount of donor fat available at other body areas. I do not know your height, weight, BMI, but with a bra size of 32A, I would not expect that you are likely to have a large amount of excess fat at potential donor sites. In order to reliably augment each of your breasts by 1 full cup size or 200cc, it would be necessary to graft approximately 400cc of purified, processed fat at each breast. With an A cup initial volume, this would likely be the limit of volume that could be accepted. To obtain a total of 800cc of purified fat, it would be necessary to suction about 1.5 liters of lipoaspirate from donor sites. With these restrictions, I find that for the average candidate for this procedure, 1 cup size increase is an appropriate expectation for a single operation. Provided that there is still an adequate volume of donor fat available, staged procedures can later be performed to achieve a greater augmentation volume increase. Again, as the volume of well vascularized breast tissue enlarges, the breasts will be more capable of accepting a larger volume of grafted fat. I recommend that you consult with an ABPS certified plastic surgeon with experience performing this procedure for a complete evaluation and recommendations on how best to proceed. Answer: Which is the best augmentation option for you? An augmentation sounds like what you may need. A photo would be helpful, but nothing is more important than an in person consultation with a surgeon. Assuming that you don't have much droopiness, it can be done with an implant, with fat grafting or with both. I think that all the procedures are excellent and it really depends on each patient's individual needs to determine which is perfect. An implant is tried and tested. Most plastic surgeons perform this surgery. In a nice shaped breast most surgeons can produce a nice looking result. Choosing the right size is the most important step where less than ideal results can occur. Fat grafting is a great surgery but better suited for patients that have a nice shape to their breast and are not looking to go that much bigger. You also need to have enough fat to harvest from another part of your body. Not that many surgeons do this procedure. It is very time consuming and obtaining the best survival of the transferred fat comes with patience and precision. Not all surgeons can produce consistent results. Make sure to go to someone that does a lot of fat grafting if you want to make it worth your while. The final choice is a combination of an implant and fat grafting. The implant provides most of the size enhancement. Fat grafting is then done to provide coverage of your breast so that it looks as natural as possible. It allows for a fuller breast without looking fake. It also can be used to give a fuller cleavage for patients that have wide set breasts. This procedure is very advanced. It is really done by very few surgeons. Even surgeons that do breast surgery as the main focus of their practise may not have experience with this technique. I love the results that can be obtained with this combo. It is ideal in thin patients that wants to be fuller breasted without the tell-tale signs of an implant. The most a woman can go up in volume in one sitting would be one cup size. You could also have a small implant + fat transfer.
