With the very thin under eye skin, fat injection may not be the best option. You may benefit from transconjunctival lower blepharoplasty with fat repositioning plus skin pinch method. Perhaps later can get filler injection if needed.
Frat grafting is an excellent procedure for filling in hollows or irregularities under the eyes, in the brow / upper lid junction, or any of the face that could benefit from additional fullness. When expertly administered the results can be smooth, natural looking and lasting. It is important to find a plastic surgeon with extensive experience and expertise in fat grafting. The proper placement of fat is essential and it is critical that fat is never overfilled.
Based on the photos provided. Your best option would be CO2 Laser treating the skin around the eyes and fat transfer to the upper cheek and lateral brow (temple region). See examples in link below. The fat transfer would restore youthful contour, just look at a portrait of yourself from your late teens or twenties. The laser would smooth and tighten the skin.
Fat transfer is probably not the best option for treatment of fine lines around the eyes.It can be a good option for midface volume restoration.As such it can help to some degree with fine lines.Crows feet are generally best treated with Botox.Direct injection with fillers into the lions can also be helpful.Fillers while temporary are more accurate and forgivin then fat transfer.For some people I lower eyelid blepharoplasty may also be a better option or lower eyelid blepharoplasty in conjunction with volume restoration.The careful examination is needed for each one of these areas and sometimes a single treatment may not be sufficient for each of these areas because each one can be quite unique.For example the area under the ice is made up of the lower eyelids including lower eyelid front pads, tear trough and midface volume issues.Each one of these needs to be addressed separately to create an accurate assessment and treatment plan.definitely wants an in person consultation to get an accurate assessment and the answer to your question.Best,Mats Hagstrom M.D.
You really don't have much of a problem under your eyes. The problem is your lower face which is unnaturally thin and made worse by your smile pattern. This is the first and most pressing issue I saw when looking at your picture. when I see people for issues like this I will inject a little local anesthetic into the areas. This duplicates the effect of the filler, like trying on clothes before your buy them. You can decide how it looks.
Thanks for your question "Bree." There are many options to help that area and your best path would be to meet in person with a Bd Certified plastic surgeon to discuss the benefits of each of them. You need function in the muscle that supports the lower eyelid and consequently you'll always have some wrinkling when you smile despite all of the treatments. Without photos it is tough but you may need to have your lower eyelids tightened with a canthopexy and then at the same time maybe do some conservative fat gr5afting to your lower lids.l
