I suggest doing as the surgeon wants as he likely knows this procedure very well if you are trusting them to do this. There are many forms of tread based neck and face lifts that keep changing names. Since introduction of sutures of any kind these lifts generally failed to produce long lasting results and turn to be waste of money and time. When someone has an anatomical problem (platysma muscle bands, external and deep fat tissue) only surgical reduction, tightening and shaping can create long lasting result. When you think about it, the bands are usually in the midline under the chin, the process of only lifting them from the sides distracts them apart further and pulls them further out of position. As far as cutting the bands, it depends where but in general, care should be taken where they are cut because contour deformities can result. Suturing the bands with medial and lateral suturing with a facelift is the most practical solution without unintended results.
