Question

Hi I was wondering about my eyes. Everyday I constantly hold my lower eye lid up like you see in some pictures. When I relax them they get big and uneven. Is this due to my cheekbones not being prominent (as some doctors have told me) or is this all in the eyes themselves? Ive very self conscious about it because when I talk my eyes sag at different lengths and its uncomfortable and unattractive. What would fix this and would it look natural? Thanks

It is difficult to tell from the photos submitted but it they right eye appears larger, either because the right eyeball is more bulgy or the right lower eyelid is retracted (lower).  Best to see an oculoplastic specialist for evaluation.

For whatever reason your right lower lid has excessive laxity.  Patients with what is called negative vector relationships between their cheek and eyeball are prone to this problem.  An in-person evaluation would be necessary to determine the correct procedure but you would likely need some form of canthoplasty/ canthopexy.

You are fairly young to be having this issue de novo. Have you had other facial surgeries/trauma or even dermabrasion? While your actual canthal tilt (line between inner and outer corners) seems to be neutral, there is no doubt that when you relax, there is more white of your eye that shows especially on the right (sclera like show). If this is causing you to change your behavior and be self conscious about it, small things can be done to both tighten up your lower eyelids. Long term, it is helpful to have cheek support for your lower eyelids. Seek an in person consultation with a board certified plastic surgeon who deals with eyelids and faces.
