If you are only allowed to lipo 4/5 litres of fat and you have that amount of fat, why do most Drs still ask you to gain weight?

Question

A lot Drs ask you to gain weight for BBL even if you have enough fat for it. Why do they ask you to gain extra weight when you can only transfer around 4/5 litres and you have more than 4/5 litres on your body? I also see some BBL surgery where it looks like more than 4/5 litres is transferred, so do they do more than one operation or is there different rules regarding lipo and BBL?

If you are underweight, then you may need to gain some weight before having a BBL procedure. You will not come close to the 4/5 liter mark if you are under weight. Consult with a few plastic surgeons in person to determine if you are a good candidate for this procedure. Also, check your BMI as this is a rough tool at determining if you have enough (or too much)  fat on your body for this procedure.Best wishes,Dr.Bruno

In almost every case, I do not recommend weight gain prior to a BBL.  I think it is better to maintain your stable weight.

astrosaz Hi There , Thank you for your Question Get in mind that only 50 to 60 % of fat survives , for a BBl you need a 32 BMI , If we remove 4 liters of fat , lipo aspirate this doesn't mean that 2 liters of fat its injected , Liposuction aspirate consist of fat , tumescent fluid , blood and interstitial fluid , so to get the best results sometimes you need to gain weight . We are allowed to remove 4-5L of lipoaspirate. Lipoaspirate is not solid fat. If we remove 4L of lipoaspirate, this does NOT mean 2L of fat is injected to each buttock/hip during a BBL. The lipoaspirate consists of fat, tumescent fluid, blood and interstitial fluid. It is my experience that when you strain this lipoaspirate after it is removed 5L produces 1100cc to 2000cc of solid fat. Patients are not created equal when it comes to solid fat percentage against lipoaspirate removed. I personally do not ask patients to gain weight for the BBL, if I feel they have 5L of lipoaspirate to be removed. Here in TX if you stay the night with medical monitoring, we can remove more than 5L (if the patient has more to remove). This mean more solid fat can be injected to each buttock/hip. When using a closed system to do the fat transfer more fluid is left in the fat that is being injected which leads to a higher number, but in reality the amount of solid fat is still the same.

If you have enough fat for the result you are looking for, there is no reason to gain weight. I only ask patients to gain some weight if they dont have enough fat to achieve their goals. Hi and Thanks for the question.  During the Brazilian Butt Lift (BBL) procedure, fat is liposuctioned and removed from areas of excess in the body and transferred to areas of deficiency in the buttocks to improve the overall, shape, contour, and volume.  One of the biggest challenges with the procedure is in obtaining enough fat during the liposuction portion of the procedure to achieve the shape and volume in the buttocks that the patient desires and it is always helpful to have as much donor fat available to transfer as possible to meet patient expectations. While each Plastic Surgeon has their own protocols that they follow in terms of whether or not to recommend that patients gain weight prior to the procedure, my own personal preference is to not recommend gaining weight but allowing the patient to do so if they desire.  As you indicated in your question, additional fat transfer procedures to the buttocks can be performed in the future after the original procedure if additional volume is desired.  Best of Luck!
