If you are only allowed to lipo 4/5 litres of fat and you have that amount of fat, why do most Drs still ask you to gain weight?

A lot Drs ask you to gain weight for BBL even if you have enough fat for it. Why do they ask you to gain extra weight when you can only transfer around 4/5 litres and you have more than 4/5 litres on your body? I also see some BBL surgery where it looks like more than 4/5 litres is transferred, so do they do more than one operation or is there different rules regarding lipo and BBL?
