Is 200 cc of fat in each butt cheek enough to make a difference? (Photo)

Question

Expert doc said I have ~ 400 cc of body fat, ~ 200 cc fat per cheek, adding 70% to mid-buttocks and 30% laterally. I want a full rounded hourglass shape. Is it worth spending $15-17k to see a difference since there's also 25% loss of fat the first 3 months? Also recommended no weight gain and sculptra 1 yr after. Should I do another combo of procedures - i.e. Lipo + sculptra (skip BBL)? I'm 5' 3", 118 lbs. Athletic. No arched back - spine lined up with pelvic bone, and ribcage = hip width.

I think you would be a great candidate for a skinny BBL.  Liposuction of your abdomen, waist/hips, and lower back to give you an hourglass shape with a fat transfer to the hips.  200cc to each side is enough to see a small difference, however, not enough for a large BBL.  I would recommend being sen by another board certified plastic surgeon who specializes in this procedure.

Thank you for your question. I disagree with your doctor. I have performed BBL’s on patients with similar body types and achieved great results. If you were my patient i would perform aggressive liposuction to the flanks, lower back, mid back and maybe even some off the abdomen. Butt do not do sculptra as a first line treatment as there is a lot someone can do with your own fat if they have the experience. I would suggest to get a few more recommendation from plastic surgeons. Most should have pictures with similar body types as we do to show you and be able to give you an idea of just what is possible. Best of luck. Have a great day.

Thank you for your question and photos. Based on these limited view photos, the sculpting part of the body is as or more important in your case than the amount of fat injected into the buttocks. Your BMI is 20.9, so you have limited fat to be transplanted. If you were my patient, I would use VASER Hi Def lipo to sculpt your whole back, waist, hips, abdomen, anterior and lateral chest wall and lateral thighs. This will give approximately 500cc/side(based on experience with low BMI patients).See a board certified plastic surgeon experienced in advanced body sculpting techniques for an in person consultation/evaluation. I have liposuctioned models and thinner patients with good results, but an exam would provide a better way to evaluate you for the procedure.  The thinner patient requires an expert at liposuction to remove as much fat as possible while leaving the skin smooth.  You may notice the enhancement you are seeking with 200cc fat SURVIVAL in each buttock if the liposuction creates more contours to give the illusion that your buttocks are larger.  You and your PS will have to determine whether the fat harvest and transfer will provide a significant improvement.Find a board certified plastic surgeon with ELITE (education, training, experience, favorable patient reviews) credentials who performs hundreds of BBLs each year.  Then look at the plastic surgeon's website before and after photo galleries to get a sense of who can deliver the results you are seeking.Kenneth Hughes, MD, Board Certified Plastic SurgeonLos Angeles, CA

Dear katkod,the cost of surgery depends on a lot of factors (type of surgery, number of treated areas, anesthesia fee, surgeon, facility fee, city, etc.). In LA area, BBL typically ranges from 8K-12K depending on how much liposuction needs to be done to get enough fat. It would be the best to visit or call plastic surgery offices and ask for a price. 200 cc should not provide significant increase in size. Regarding the result, most patients lose some fat after transfer and you will be able to see the final results few months after surgery. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Patients interested in BBL often ask questions of the necessary volume for transfer. Ultimately, the answer is two pronged and based upon both patient expectations and the nature of fat graft themselves.
BBL and fat transfer is ultimately dependent upon the long term viability of the fat grafted. That being said the resorption rates of fat transferred can be significant. Clinically this translates to a progressive loss of grafted volume in the months following the procedure.
I tell patients that they can expect resorption or graft loss on the order of 50% (at most; prepare for the worst and hope for significantly better). Additionally some harvest volume is lost in processing the fat graft prior to transfer. Thus I generally recommend that patients seeking even the most minimal volume increase have adequate reserves for grafting of 500 cc per cheek (at minimum).  This means that harvest will usually exceed 1 liter (assuming for processing loss).
However, it important to remember that these are recommendations and can be amended based upon a patient's particular expectations. Some patients only want a subtle change and may be willing to accept a less significant result. But the rule still applies you have to have the fat to transfer the fat.If your harvest yields 200 cc/cheek, after adjusting for resorption/volume loss, the yield will be smaller. For comparison the volume of a shot glass is 44 cc. A loss of half the 200 cc volume would amount to a fat yield of little more than 2x that volume.Alternative options include Sculptra (which can be costly over time), implants (which will ensure greater volume but with all the drawbacks associated with an implant), EMSculpt (non-invasive muscle stimulator, can be used in conjunction with other approaches to build muscle mass and subsequently volume).

In order to determine the best course of treatment, I would first need to know what your goals were and if it was realistic. You would be a candidate for lipo and BBL but the enhancement of your butt would be limited. Even so, if you were having lipo, I would use whatever fat I had, and do the BBL to try to achieve the best result. I dont think sculptra in your butt would be worth the cost. 200cc to each side is enough to see a small difference. 200cc will not give you the typical hourglass shape that you probably desire.  If you are going to do the lipo, definitely transfer the fat. Sculptra is expensive to get even 200cc to each side, so best to place as much fat as possible.
