Many times, alar base incisions may initially have a small separation, or dehiscence.  These usually heal well over the course of a few weeks.  In the event that they do not, they are simply corrected under local anesthesia in the office.  Avoid smoking, maintain adequate protein intake, and apply antibiotic ointment in the meantime.

The incision looks to be healing well and there is just slight opening on the right side. This is not uncommon and usually heals well without need for revision of the incision. I would recommend applying aquaphor, neosporin or bacitracin ointment two to three times day until you see that it matches the other side.
