Hi.  If you are a 40DDD and want to be a 36D, you are asking to be significantly smaller.  What you are really asking for is a breast reduction, or a lift with a small reduction component.An implant would make the breast larger, and would be taking you in the wrong direction.

A breast lift operation would be an excellent choice.  There is absolutely no reason why you would need an implant with the lift.  Find a surgeon who has photographs on their website of lifts you like.  The boxy look is a problem of planning.

A DDD to a D is a reduction in size, so an implant would not make sense.  What you probably want is perkier, rounder looking breasts.  This certainly can be achieved with a lift alone.  The boxy appearance you refer to can be avoided with the right technique.  An implant can always be added down the road if you so desire, but cup size is determined by the volume of the breast and you have DDD breasts.

It sounds like you would do well with either a breast lift alone, or a breast lift and a small reduction. An augmentation would increase the size of your breast, would add increased weight to the breast, and would cause your breasts to droop/sag more quickly after surgery. A breast reduction or lift would elevate your nipple to a more elevated position, and reshape the breast to give the entire breast a more youthful appearance.Consultation with your surgeon will help you determine how best to obtain your goals.
