I’m 51 yes old, 5’9” and 185#’s. I had a sleeve done 3 years ago and a revision to bypass 8 months ago. My initial weight was 292 and a goal weight of 165-170. I desperately need to have the skin removed that is hanging. It hinders exercise, movement in general, and I am constantly fighting yeast rashes below my panni. I am not sure what surgery would help me the most. At my age I am not concerned about what I look like naked but this hinders wearing normal clothing even. What do you suggest?

A panniculectomy is a surgery done to remove excess skin and fat from your abdomen after weight loss surgery. It is often times covered by insurance, though abdominoplasty and lipo suction is not. I would recommend getting an Abdominoplasty as that gets rid of any excess abdominal skin that you have as well as includes liposuction of the abdomen/mons area and tightening of the abdominal muscles (diastasis recti). All that is done in one procedure and will be most beneficial for you and your goals. I would recommend consulting with a board certified plastic surgeon like myself so we can create a surgical plan.

Answer: Body contouring surgery after massive weight loss Hello and congratulations on your weight loss journey! There are many options when it comes to body contouring after massive weight loss (100+ lbs). In general, for the best results we want patients to be at or as close as possible to their goal weight. Based on your photos available I would recommend a fleur-de-lis abdominoplasty to remove the excess horizontal and vertical abdominal loose skin. The trade off on this operation is accepting a vertical midline scar in addition to the low horizontal "bikini line" scar. A traditional full abdominoplasty would avoid the vertical midline scar but would likely leave some upper abdominal skin laxity in a horizontal/side-to-side dimension. You would likely also benefit from liposuction to your flanks to further enhance your contour as well as a breast lift/reduction. We are unable to see your back, but it is also common to need to address the posterior trunk in a 360 degree manner with an upper back lift/gluteal lift operation at the same time vs another surgery. As always, I recommend an in-person consultation with a board certified plastic surgeon to discuss your specific history, exam, goals and questions.
