6 weeks post-op I have really hard tissue still feels like gland on the left lower outside of left nipple, doctor says it’s not tissue tells me to massage it but that just irritates it and seems to make it worse, other side is completely flat, I did have the drain in on that side for a whole week due to excessive bleeding but how could scar tissue form that fast? It’s only noticeable when I put my hands up and sticks through my shirt.

It is hard to tell by the photos, but a mammogram "might" show the discrepancy between the right and left breast.
If indeed there is too much a depression on the left, then an autologous fat grafting procedure can equalize the right and left breast. Good luck!

Based on your photos, it appears you have extra skin on the left, and to achieve a contour similar to the right, a skin resection would be advised. Hope this helps.
