You don't "have to" take finasteride after a transplant (no one can force you) any more than you "have to" do maintenance on your car, house and other belongings, but your long-term results will be much better. 

It's not outright required, but most doctors won't perform a hair transplant on you without knowing that your hair loss has stabilized, either naturally or with the use of Finasteride/Propecia. Additionally, the medication can help prevent shock loss which will make your end result look better sooner. 

It is not a requirement to take finasteride after a procedure, but it is recommended to help prevent future hair loss. If you decide not to take it you may have to have an additional hair restoration down the road as your hair loss progresses.
