Question

Wondering if these look like normal scabs or can it be the beginning of necrosis? I am 23 days post op mommy makeover. Not a smoker but have been around second hand smoke twice since recovering.

Based on the submitted photos it appears you have normal scabs from healing.  The incision and surrounding tissue looks to be healing appropriately.  Closely follow all post op instructions and visit with your surgeon as advised.

Thank you for sharing your question and photos. It appears that you are on track with healing, with no necrosis. You can use Neosporin on the area for a few days (about 4 or 5 days), or Aquaphor. After you have applied the Neosporin or Aquaphor, cover the area with gauze to avoid friction from clothing. It is best to follow your Plastic Surgeon's instructions for post-operative care, and be sure he/she is aware of your concerns. I hope this was helpful to you.

From your photos everything looks normal. You can put something like neosporin or bacitracin over the incision line daily for ~7 days to help things heal. Do not pick the scabs as this can lead to an uglier scar. Let them fall of on their own in the shower when you are washing.

Tummy tuck healing is a long slow process. The edges of the skin on the upper portion of the scar are thicker and more swollen and have less circulation than the lower skin edges. Lumps, bumps, scabs, itching, patchy redness, occasional fluid leakage, and areas of numbness are all normal and can take weeks to months to resolve. Keeping the skin clean and dry is important to prevent infection, so patients need to wash their garments and carefully wash and dry their skin. We also encourage our patients to add a type of lotion or oil with light massage to their healing process after body contouring surgery.

These scabs look like a normal part of the healing process and do not seem to be showing any apparent signs of necrosis.  Be sure to be wearing paper tape over your incisions as close to 24/7 as you are able to; replacing the tape every 5-10 days.  This will help to protect the scar from friction from clothing and will help to keep the incision covered and protected.  I have my patients wear tape over the incisions for 6 months to a year following surgery in order to reduce the size and color of their scar. Continue to follow the recommendations of your plastic surgeon.
