Based on your description, this might be fat necrosis. It is best that you visit your plastic surgeon for further assessment. Thank you for your question.  It is possible that what you are describing is an area of fat necrosis.  This can occur when some of the fat that is transferred into the buttock does not survive and the fat cells die.  Most of the time, these dead fat cells are absorbed by the body without any consequences except a potentially smaller final size than desired.  However, in some cases, the dead fat cells are lumped into a ball and instead of being absorbed, scar tissue forms around them.  This can result in a firm and often hard bump.  These also can be painful, especially to sit on.  Sometimes the dead fat cells in the center of the ball liquefy so that the feel of the ball instead of being firm is softer.  These softer balls of scar tissue can still be painful.  The first step is to call your plastic surgeon and discuss what is occurring.  You may need a follow up exam to determine the cause of your discomfort and the possible solutions to treat it.  I hope this helps.
