A bit annoyed that I payed money for less than agreed. Is there is much difference between 410cc and 450cc?

Dear Ripkita, I am sorry that you have had an unpleasant experience with your breast augmentation surgery.  The difference between a 450cc implant and a 410cc implant is 40cc's which if translated into what I often call "American numbers" is around one and a third ounce of fluid, or slightly more than a small shot glass.  This volume difference between the two implant sizes you selected usually does make a large difference in the size or the appearance of the breasts after surgery however you should have an explanation from your surgeon why they selected to use the smaller implant size.  There are many good reasons why surgeons sometimes select to go a bit smaller than originally planned.  Please meet with your plastic surgeon and ask they schedule you with enough time for you to address your concerns.  

As far as a "Size 8", in the world of breast augmentation, there is no reference to this measurement.  Another question to ask your doctor.

How soon after pre-op can I schedule my BA?

Thank you for your question.  Every plastic surgeon has their own set of pre-surgery requirements so there is no one correct answer.  Also, the pre-op testing requirements vary from center to center and state to state.  So check with your plastic surgeon's office to see what pre-op requirements are needed before your surgery.

In my office, since many of our patients travel from long distances for their surgery, we have endeavored to make the process as easy as possible.  For a healthy patient under 50 years of age and no unusual medical issues, we only require a hemoglobin test performed in our office.  This way, many patients can have their consultation, followed by their pre-op sizing and testing, and then have surgery in just a few days.  We also have a large implant consignment so it is a rare event when we do not have the size and style the patient desires.

Can my ideal breasts be achieved with round implants?

Thank you for your question.  It is one we get asked quite often,  There used to be a running joke that to start a fight at a meeting of plastic surgeons  all one had to do was ask which implants are better-round vs. teardrop.  There is no true correct answer but the trend today is more towards the round smooth implants.  

The newer generation round silicone gel implants offer different degrees of firmness in the gel filling materials which can make the implant more mobile and soft or less mobile and firmer.  Many patients patients can achieve the look of a teardrop implant without the fear of rotation.

Teardrop shaped implants are almost all covered with a textured surface to minimize the risk of implant rotation.  Texturing of implants is "suspected" of increasing risk for developing BI-ALCL.  And even though those risks are very small, patients are moving towards using smooth wall breast implants instead of texture.

Based on your photos, you appear to be an excellent candidate for an augmentation.  Based on your tissue characteristics and your examination, your plastic surgeon should be able to select the right type and style of implant to help you achieve your desired goals.  I think you can achieve that look with a round implant.

Do most providers offer nurse discounts?

In my practice, I do offer discounts for nurses, military, and first responders.  They take care of us, I take care of them.  I know of a number of other plastic surgeons who also offer discounts. Of course, every practice varies and not everyone does follow this practice.

Am I good candidate for fat transfer? (photos)

Thank you for your question about having a Brazilian Butt Lift (BBL).   From your photos, the ideal combination of procedures would be a BBL combined  a tummy tuck and liposuction of the abdomen and love handles.  This combination would increase your buttock size while tightening and shaping your waist and abdomen.  If you did a BBL with only liposuction, the skin and remaining soft tissues of the abdomen will probably not shrink back enough to avoid having loose, hanging skin around the waist line.  For some patients, because they cannot have the tummy tuck at the same time as their BBL,  they will have the liposuction and BBL first and then return later to have their tummy tuck.  Their results are just a good as having all 3 surgeries at the same time.   Your surgeon will be able to work with you to fit your surgeries  into your lifestyle needs.

Please consult with a Board-Certified plastic surgeon who follows the ASAPS/ASPS recommended  guidelines for fat augmentation of the buttock.

How to know I have a yeast infection after surgery? I have minor sporadic itching, thick white discharge and an off smell.

Thank you for your question.  Yeast infections after a labiaplasty are not uncommon.  The cleaning and the preparation of the vaginal region and any antibiotics your surgeon may have placed you on before, during, or after your surgery can alter the normal bacterial and yeast environment in this area.  While itching after a labiaplasty is very common, a thick white discharge with a foul odor is not.  Oral diflucan can rapidly reduce the itching and the discharge.  If your plastic surgeon cannot respond, your OB-GYN can certainly assist you with this problem.  

Ice packs and iced Tucks pads can bring relief also from your symptoms.

Can I go in pool after my labiaplasty?

Thank you for your question.  The only person who can truly answer your question is your surgeon.  They know your surgery and how your recovery is proceeding better than anyone.  They may have provided guidelines for your recovery at the time of your surgery.

Generally in my practice, I allow showering almost immediately.  Patients can sit in a pool, hot tub, or bath but not soak for long periods at 4 weeks.  If swimming, I ask them to go to the dressing room, gently dry the labial area and wring out their swimsuit  to avoid having a soaking wet garment on the recent surgical area. All restrictions removed at 6 weeks.  These recommendations though are modified to fit each patient's individual rate of healing.

1 week after labiaplasty. Bleeding, splitting, and hematoma. What should I do in this situation?

Thank you for your question.  Anytime something unusual occurs after a surgery, I recommend you contact your surgeon.  Most surgeons are reachable 24 hours a day.  Because this could be a hematoma, it needs to be evaluated.  Hematomas can lead to breakdown of the suture line with wound separation.  Infection can also occur.  Proper treatment needs an examination and evaluation by your surgeon.  Please call them today.  Sincerely, Herluf Lund M.D.

Would a dose of aspirin close to surgery be a problem?

First, thank you for being upfront and asking about your accidental aspirin usage.  It is always better for your plastic surgeon to know about it ahead of time rather than after.

: Call your plastic surgeon's office today and tell them what occurred

: They most likely will not cancel your surgery but you are at higher risk for bleeding, the need for drains, and increased bruising.   Better to be aware of this before your surgery.

What could be causing pain underneath the areola 2 weeks & 2 days post-op? Only in one breast. (Photos)

Thank you for your question.  It is one plastic surgeons often hear after a breast augmentation.  Nerve hyper-sensitivity or hypo-sensitivity during the healing process is common and maybe due to a number of factors including:

: Stretching of the nerves from the implant or the surgery to place the implant.  These nerves will recover as the tissues relax from the stretching.

: Dividing of the smaller nerve endings by the incision.  Since these nerve endings are microscopic and random in their pattern (right and left sides can differ), one breast may be more sensitive or numb than the other side.  These nerve endings will regenerate and heal but can take months to do so.

Treatment that can decrease your discomfort include taking medications, commonly a NSAID such as Motrin, for a short time.  Wearing a well-fitted, supportive bra without pressure points digging into the breast.  This may include sleeping in a bra at night for a period after your surgery.

Before beginning any treatment, contact your plastic surgeon's office and discuss your concerns.  They know your case best and can give you the best idea about the time for your recovery.

I am super satisfied with the procedure that Dr Lund performed. I had a breast augmentation and I am thrilled with the results. Everything went just perfect. Dr Lund is an excellent, very knowledgeable and experienced surgeon. His staff and nurse Andrea are just on top of it. Very professional staff which gives you so much confidence. I am also was extremely happy with Des Peres surgical facility. My nurse was very attentive and was offering socks and new warm blankets constantly which I really appreciate because I was a little nervous. Anesthesiologist was very knowledgeable and super nice explaining whole process to me. I wish I could done it earlier. I am insanely happy right now because I love how my whole body looks with this change. I can wear clothes that I never even was considering trying on in the past.

I can not recommend Dr. Lund, his nurse Andrea, and the entire staff at St Louis Cosmetic Surgery enough. Dr. Lund is incredibly talented with a scalpel- in my wildest dreams I couldn’t have imagined the results he delivered. He was very knowledgeable and up front throughout the entire process. Well here I go again. If you read my last review you will understand lipo did not work for me in the long run. most likely because of my age and skin elasticity Since my last treatment did not last very long I figured I would get my eyes done at the same time as my tummy tuck with muscle repair. I have had sunken/ dark under eyes for years. The face pics are with no make up. I will try and put up a couple with makeup before surgery. Surgery is March 17, 2022 Yea!!! St Patrick’s Day. Age, time, and gravity can be unforgiving. 56 yrs. 5”8 164 lbs. I was down to 154, but than loose skin really shows. Not an excuse to gain just a fact Updated on 17 Mar 2022: Well here I go. I will try to post as soon as Able. I’m a bit nervous, but overall feel pretty good. I will not give a Doctor name or review until I have had a chance to heal a bit. Overall the staff has been wonderful, and my doctor has been very helpful and informative with what I am having done. Happy St Patrick’s Day !! Updated on 20 Mar 2022: As stated below It has been 4 days since surgery and so far so good. Not hungry because of the wrap around my middle, but forcing some food into myself because of the meds I’m taking. I was pretty nauseous first couple days. I ate mashed potatoes with blue berries Sounds awful but it stayed down so that is what I ate for 2 days. Also bit of ice cream with the juven packets and arnica pills. Updated on 23 Mar 2022: All is well. Everything is healing nicely. I’m only posting eye pics for the moment since it is very hard to take clear pics when my eyes are still healing. Drains are out. Wasn’t painful, and upper eye stitches are out ( just a tad bit painful, but after all the back spasms piece of cake IMO). I could not handle the pain killer made me want to puke even half a pill. Diazepam and Tylenol, and celebrex has worked for me. Updated on 27 Mar 2022: 10 days since surgery. Still very tight and sore but softening up a bit each day. Not happy with belly button, but hoping the healing process will improve with time. Very lumpy under skin, so I am doing light massage to try and soften up tissue underneath Updated on 1 Apr 2022: So it has been 15 days since surgery. I am still a bit puffy I think, but I am happy with results so far. Under right eye is still sore to the touch Dr said to use warm compresses. I am doing that throughout the day. I had son take pics today I have no filter and no makeup on at all, but I think his phone takes better pics than my old phone. As for the ab muscle repair diastisis recti pain comes and goes which I expect to happen for quite awhile. Hoping to return to work at 8 weeks post op. Updated on 2 Apr 2022: I wanted to add side by side for my 2 week update yesterday, but I had no clue how to do a side by side. Hahahaha yeah I had to have my daughter show me. Lighting is in different houses, time of day and phones, but I don’t have makeup on in any of them. Figure while I’m off May as well not wear it. When I go back to work it is an everyday inconvenience. Makes us look better IMO, but maybe now I won’t have to spend 10 min trying to hide dark under eye circles. Updated on 9 Jun 2022: Very happy with results. I still have some pain at the top of my abs where stitching is I believe. Also I have small spot that keeps opening up on incision. I still swell at end of day. As for my eyes. Super happy I barely have to where any makeup when not at work, and eyeliner no longer ends up all over the place. Stays where I put it, but I don’t where much of it. I feel it ages me. Updated on 25 Nov 2023: All is well. Still very happy with results it has been about 20 mths. Dr Lund did an amazing job. Hoping to get lower face done in the future, but as of now still saving up for that. I turn 58 in December, and still wear a bikini which is a plus in my opinion.
