You have what is known as a "double bubble" breast issue, which happens when the breast implant falls below your normal breast fold. This leaves your natural breast sitting above the implant with an unnatural crease or fold.

The fix is to move the implants higher and reconstruct the breast implant pocket. Sometimes the size or profile of the implant also needs to be changed.

There are several "issues," the most obvious being a visible linear differentiation between your implant(s) and your native breast(s). What we see is a classic double bubble deformity where the line of demarcation medially in the picture is where the old inframammary fold was attached to the chest wall. During the breast augmentation phase of your original surgery, the inframammary fold was probably released and the implant has settled lower than the previous fold. Although you're saying the original surgery was in 2020, this could have been unmasked by recent childbearing or a loss in weight that made this unappreciable prior to now. To repair this deformity will require the previous inframammary fold to be reestablished and control the location of the pocket within these anatomical limits. It is possible that a smaller implant will be required for a time until this fold is healed. Most plastic surgeons would now employ a technique of using internal mesh to secure the inframammary fold and delineate the pocket to an appropriate position. My recommendation is you seek consultations from an experienced plastic surgeon who is comfortable utilizing mesh to repair and define implant pockets.

Based on only one posted photo it appears you have a double bubble deformity of the left breast. You also only had a donut lift with the implants, did you have a sub muscular placement of the implant? It appears you did therefore after first failed the temp of fat grafting without internal release, I would recommend changing the location of the implant to the Dual plane location. You have capsular contracture. And you need a surgery to fix this. Capsul should be removed and implants should be changed.
