Hello,You got a 'closed rhinoplasty' for certain, and therefore it is likely that you got a 'reduction rhinoplasty' only.  That is to say, the surgeon probably only removed tissue, and did not reinforce the remaining tissue with rigidly fixed cartilage grafts or reshape your native cartilage with sutures. This has lead to a loss of function (diminished airway) and less than normal looking form (alar arching).  You could benefit from a revision rhinoplasty where cartilage is harvested from your septum and utilized to reinforce your existing cartilage as well as augment areas of laxity that allow collapse (internal valve). Additionally, a portion of your tip cartilage complex could be repositioned, which could have a dramatic effect on alar position and shape, as well as improving airflow (external valve). Thank you for the question and some very good photos.  There are two issues.  The first is the cosmetic or aesthetic result and the second, no less important, is the functional result.  The airway is important.  Ultimately a face to face consultation with a plastic surgeon (or facial plastic surgeon) can give you the most appropriate response.  Cosmetically, you have an excellent result.  There is always a deconstructionist that can point out this and that not being perfect, but remember if you do go for tweeking the result to get minor defects repaired, you risk getting a very unnatural outcome.  The second issue, a plugged nose, may be due to scarring.  Sometimes this is a very minor fix, sometimes it means a more extensive approach of placing grafts to open the airway.
