Question

got my rhinoplasty done over two years ago. Did my research & found a reputable doctor that owns his own clinic in Toronto. procedure cost $10,000. I had a typical middle eastern nose, slight hump & drooping of the tip. He shaved the bump and raised the tip. That's all that he said he would do. Now I have troubles breathing, you can see up my nostrils into the inside of my nose, the tip looks/feels Square shaped rather than round, The bridge still has a slight hump (should have more of an arch)

From your photos it appears you have an increased columellar on the right. It's quite possible that your internal valve has been narrowed too much causing you breathing difficulties. These are aesthetic considerations that you need to discuss with your surgeon who might be able to offer you a revision surgery.

No, your results are not what you like and a revision rhinoplasty done by a very experienced revision rhinoplasty surgeon can get you a nose closer to what you want. All rhinoplasty surgeons have to deal with scarring after surgery which can alter results.

Hello,You got a 'closed rhinoplasty' for certain, and therefore it is likely that you got a 'reduction rhinoplasty' only.  That is to say, the surgeon probably only removed tissue, and did not reinforce the remaining tissue with rigidly fixed cartilage grafts or reshape your native cartilage with sutures. This has lead to a loss of function (diminished airway) and less than normal looking form (alar arching).  You could benefit from a revision rhinoplasty where cartilage is harvested from your septum and utilized to reinforce your existing cartilage as well as augment areas of laxity that allow collapse (internal valve). Additionally, a portion of your tip cartilage complex could be repositioned, which could have a dramatic effect on alar position and shape, as well as improving airflow (external valve). Go visit a few surgeons who specialize in structural rhinoplasty. Best of luck!

Many rhinoplasty operations go well and then begin a process of healing that is difficult to explain.  So it is not that your nose is botched per se, but its not a result that pleases you.  This is where revision rhinoplasty begins to play a role.  Find a good revision surgeon to assess the situation and look at your options.

Thank you for the question and some very good photos.  There are two issues.  The first is the cosmetic or aesthetic result and the second, no less important, is the functional result.  The airway is important.  Ultimately a face to face consultation with a plastic surgeon (or facial plastic surgeon) can give you the most appropriate response.  Cosmetically, you have an excellent result.  There is always a deconstructionist that can point out this and that not being perfect, but remember if you do go for tweeking the result to get minor defects repaired, you risk getting a very unnatural outcome.  The second issue, a plugged nose, may be due to scarring.  Sometimes this is a very minor fix, sometimes it means a more extensive approach of placing grafts to open the airway.

your nose does not look botched but there are some features such as the fullness of your profile and the retraction of your nostril rims that you may want to change. It will be necessary to examine you to determine the cause of your breathing obstruction. See an experienced revision specialist.
