I appreciate your concerns and fat grafting to even out any irregularities is certainly an option. However, sometimes scar tissue beneath the skin which is causing it to adhere to the muscle has to be released also. it is hard to tell for sure without an examination, but in most case sit can be improved with fat transfer. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

What you are describing is referred to as aninmation deformity. This is caused by excessive removal of breast tissue and fat in the area where deformity occurs. The scar tissue which forms to heal and surgical site tethers the area of over-resection to the pectoralis major muscle. So, when the muscle contracts, it pulls on the scarred tissue almost like a marionette string. The only way of correcting this is to release the scar tissue and place grafts of solid fat or fat and dermis between the skin that is released and the muscle. Animation deformity after poorly performed gynecomastia surgery can be very difficult to correct and in some cases, may even require more than one surgical procedure depeneding upon the severity of the situation.  I urge you to find a plastic surgeon with extensive experience in gynecomastia correction surgery.
