Can the closed technique "rhinoplasty" straighten the columella? (photos)

The techniques used is based on the surgeons skill, comfort level or preference.  Either techniques in the right hands can achieve the goals you aim for.  Expertise and experience in rhinoplasty is most important.

The short answer is yes. Your septum is causing your columella to be crooked, so straightening that will help. Now whether you should have open or closed rhinoplasty is dependent on your surgeon and you together. I think access to your nose and graft placement might be better in an open approach.  The incision heals great. There is a little more swelling initially but that also goes back to the level of closed rhinoplasty with a few weeks!   Hope this helps!

Both techniques can accomplish the goals of straightening the nose. The only difference is the incision across the columella. There is plenty of exposure  accomplished with closed techniques to accomplish straightening of the nose.  Osteotomies of the nasal bones will be required to straighten them, a spreader graft composed of cartilage was need to be inserted underneath the concave upper lateral cartilage in the mid vault area, a  tip rhinoplasty will be needed as well as a columellar- aplasty. The nose - including the tip and columella - can be straightened endonasally (with hidden incisions) or externally using an open incision - really the best way depends on the comfort level of your surgeon and his or her experience with either technique.
