My chin is recessed and i Have lip incompetence , i Would want to know what type Of mouvement should i Have , according to the 1st surgeron i should move my chin forward 10mm and upward because my chin is long and recessed and according to the second surgeron i should just live it forward because according to him it Would be unesthetical to live upward , could someone tell me what should i do?
Answer: Forward with a long backcut You need to advance and decrease lower face height. The hard part is having a steep mandibular plane like yours, you are prone to having a bump at the junction of the osteotomy. If you do a long back cut by the 2nd bicuspids and go straight forward or even up a bit will help minimize it. I also prepare patients for possibility that we need to contour it down after it heals, which we usually do along with removing the hardware. For a number of patients they can feel it but not see it, and they may choose not to contour it. You may want to think about augmenting the angle with implants for improving overall facial balance, defining the jawlne, and decreasing the angle of the mandibular plane.

Answer: Genioplasty movement You can definitely benefit from forward advancement. It seems like everyone agrees on that. The vertical height of your chin is definitely there so some slight upward movement would reduce that to some degree. It would make your whole head in the face less tall in relationship to the width. Consider finding a provider who has software morphing program or try to find an app or one online where you can manipulate the shape yourself and get an idea if you think adding a little upward motion and reducing the vertical height of your chin would look better. Include your whole head and face in the image since this is a matter of balancing. There’s nothing wrong with having more than two consultations. During each consultation ask each provider to open up their portfolio and show you their entire collection of before and after pictures. Ask them to point out previous patients who have similar facial characteristics. Ask them to show you pictures of patients who had just forwarded management or forward and up movement and perhaps you’ll be able to see the difference. Push providers to show you as many before and after pictures as possible including examples of excellent outcomes, average outcomes including outcomes that did not turn out as well as they had hoped for. Ask them specifically what the revision rate is and what their revision policy is. Do you want to get a really good idea of what the average results look like and how many of these procedures each provider has done in the past. I have a more consultations you’ll start seeing a trend in regards to recommended treatment and if you consistently hear the same thing then that’s probably the way to go. Having multiple consultations typically makes patients better consumers because you understand the process and you get better at vetting your provider more thoroughly asking them more appropriate questions and especially pushing them to open up their entire portfolio of before and after pictures. When in doubt slow down and schedule more consultations. Answer: Genioplasty They could both be correct in some way. You have a steep vertical growth pattern so the jaw is typically 'thin/narrow' from the frontal view which is not shown. One has to be cautious with the genioplasty in that regard. Certainly youbshould shorten the chin but you may need to augment the "sides" of the jaw as well to properly blend in the chin position to the narrow jaw. This avoids a 'chinny" look.
