Assuming this is not some hypothetical question, the answer really depends on a number of factors. Some transplant recipients do amazingly well, living essentially normal lives. Some are really not healthy and take a wide array of drugs to keep the transplanted organ from chronically rejecting. The two situations are vastly different. So personal motivation is a very important issue as is overall health status. These types of decisions must be done on a team basis with involvement of the various doctors involved in your care.
A blepharoplasty can be performed on a kidney recipient as long as they are in good health and the patient obtains clearance from their nephrologist. It is also important to make sure on physical exam that there is fatty deposits that need to be removed and not having abnormal swelling from kidney disease.
Lower eyelid surgery would be contraindicated if the kidney transplant has caused fluid retention around the eyes and not fat. If fatty tissue around the eyes is causing the appearance of an eye bag, then blepharoplasty would be indicated.
The key factor is whether your nephrologist would clear you for surgery and what anti-rejection drugs you are on. This is a very individualized issue and each transplant patient has his or her own medical issues. Blepharoplasty is performed under IV sedation at most. Alternatives would include Restylane or Selphyl for lower lid rejuvenation.

As long as your transplant is functioning well and your nephrologist/transplant team concurs, there should be no problem having eyelid surgery. Even though you are on some level of immunosuppression, eyelid surgery is one of the best healing areas on the body after surgical manipulation.
