Thank you for your pictures and questions. It looks like you have completely reasonable outcomes from your surgical procedures. What may not be reasonable is your expectations. Every person has a unique anatomy and your bone structure may not be able to be transformed into your ideal results. If the waist was smaller at one time, the waist can likely be reduced through liposuction of the abdomen, flanks, back, and love handles to those same measurements or smaller provided the muscular and fascial integrity has remained the same.  The harvested fat can be transferred to the buttock and hips to make your waist look even smaller. 
 The results depend very little on the method used and very much on the board certified plastic surgeon selected.

Dear, to achieve an hourglass figure surgically, we can transfer fat from waist to hips, take excess skin out and repair muscle recti diastasis. There are two structures that we can't improve surgically: bony framework and intraabdominal fat. Although both are genetically influenced, working out at the gym slowly improves both (your figure 1). Keep doing your good work!

To get a snatched waist you need to have aggressive fat removal of the torso/sides and have fat injections on the hips/buttock to give the illusion of an S Curve. This cannot be done with smart lipo, and this cannot be done awake to the full extent that contouring can be performed. I suggest you see a well trained expert in body shaping. Best, Dr. Emer.

Based on your photos, you look like a good candidate for a BBL. Liposuction of your abdomen, back and flanks should give you more curves and hourglass shape and fat transfer to buttocks should give you more projection. I would suggest you to consult a board-certified plastic surgeon because only after a thorough examination you will get more information and recommendations.

Without in person examination to see if you have any residual donor fat for transfer to lateral hips/buttock can not offer surgery. Might consider non surgical BBL with Sculptra injections or combination limited available lipo with fat transfer + Sculptra injections to laterals. 

There are a number of ways to achieve a slimmer waist. Furthermore, there are many body regions that go into a slim waist. VASER liposuction of the back and flanks can help contribute to a slimmer waist. Often times patients seek a BBL to achieve a slimmer waist as well. After looking at your pictures you appear to be a great candidate for high definition VASER liposuction. You had a good outcome from both procedures.True hourglass figures are exceedingly rare and the number of photo shopped images floating around the Internet way beyond peoples imagination.I’ve only known one surgeon who is able to truly pull in the waste with a full tummy tuck.He does muscle tightening to the point of bending women’s ribs. his patients lose 10 pounds afterwards because they can’t eat after the procedure.your results look good and you should be happy with the outcome.i’m sure there are lots of people who wish they had lots of things like men who were hung to their knee.I have certainly met enough patients in my practice to know some people are never happy no matter how good the results are.Best, Mats Hagstrom MD
