Malar bags look full blown after Restylane injection. Will this go away?

I am sorry you are having such complications from your treatment.  Hyaluronic acid fillers are known to attract and hold onto water in the tissues, which may be what you are experiencing.  I would suggest having them dissolved completely and having Sculptra treatments in those areas that need volume.  Sculptra is a PLLA (poly-L lactic acid) that is injected to stimulated your own body's natural collagen.  I suggest seeing an experienced practitioner who specializes in facial aesthetics for best results.  Good luck!

THe festoons are very difficult to treat and even more difficult to treat with fillers. Combination fillers and lasers has helped my patients to somewhat resolve their festoons. But if you just getting Restylane for tear trough area, the festoon will get worse. I usually give my patients 20 mg of Prednisone to resolve the swelling faster. and usually the area will respond to steroid one dose treatment.

The service was likely not done optimally to begin with and as it ages, it can be even more uacceptable. This product should be fully removed with hyaluronidase. I suspect you need something like 300 units of enzyme to accomplish this. Who does this removal makes a difference. Likely you might also benefit from judicious replacement but these are fussy services and it may be that in your area, there are not injectors who can accomplish this treatment for you.

This often happens with a cannula technique which is why it is not recommended to use for under-eye regions. The location of the cannula can never be precisely known so the precision and accuracy are low. In this case, there is filler placed superficially and it needs to be dissolved, it won't go away on its own.

Hi thank you for reaching out.  I am very sorry you are not achieving your desired results.  Restylane is an HA and does indeed retain water.  I think the best course of action for you is to dissolve completely and utilize a collagen stimulator such as Sculptra to fill and general your own collagen over a period of time.  Depending on your evaluation you may need multiple vials and have to be in an annual or semiannual maintenance.  I hope this helps and good luck.

Restylane and all the other Hyaluronic acid (HA) fillers such as Juvederm, Vollure, Volbella, Voluma, Belotero and Silk promote water retention especially within this treatment area, which in turn often exacerbates swelling. If the swelling remains after two weeks post treatment then its good bet the HA filler is holding onto excess water and the filler gel needs to be dissolve with hyaluronic acid.
The best thing an experienced injector can help you answer is the simple question which is " are you a good candidate for fillers for malar bags?" This area more than any other area needs to be assessed by an experienced injector because truth be told many people simply have too much loose skin and lost elasticiy to get good results.

I think if there is any concern about residual filler causing worsened appearance of the malar bags, I would recommend dissolving the filler and allowing for any residual swelling to go down before deciding on the best next step.
