I would suggest a lift. I would also suggest selecting a lower profile implant as you do not need a full profile implant. Full profile implants have a very narrow base diameter for how much projection they provide. Most lift patients need a lower profile implant that is "taller" to restore upper pole fill.
Most women prefer for their nipple position to be higher than their inframammary fold of the breast. With that being said, a lift is recommended. You could opt for a benelli lift that only goes around the areola or a traditional mastopexy for even more lift with the lollipop scar. You gave Grade I breast ptosis and could do either procedure. With just the augmentation you'd want to do a Dual plane III. You'll be perkier with the lift but will have the extra incisions/scars.
If you do an implant alone, you would need the dual-plane technique. You will get more fulness and roundness, and of course increased size.You do have a bit of a long breast fold and low nipple position (which would need to be measured on examination) and a lift would correct those issues. If you're willing to accept the lift scar, then lift with a bit smaller implant may be the way to go.Get a few opinions in person, as lift + implants is a tougher operation to get good results with, and requires a surgeon who has experience in this area.
If you get implants only your breasts simply won’t be right. You do need a lift. However bear in mind that many surgeon will tell you that a lift isn’t needed and convince you that implants alone will suffice. You need to find someone who is a breast lift specialist. If you’re not willing to have a lift you would be best off just leaving them alone
You definitely need a breast lift. The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Yes, you need a lift or you will just have larger and still sagging breasts. Implants alone will not correct the sagging.
You need to go smaller. Since your implants have bottomed out, they will do so again, even with mesh. You may need to go down to 500-550 so they will stay up longer. Breasts do not belong under your chin.
