Maneuvers for your surgery would include nasal tip deprojection and placement of a mastoid fascia tip graft to provide a smoother tip contour. Microdroplet Silikon-1000 nasal injections may be considered for camouflaging visible tip cartilage after previous rhinoplasty. It appears that you have developed nasal bossae , those are likely alar cartilage protuberances . unfortunately to correct the issue you will likely need a revision rhinoplasty.I hope this helps, best of luck!
