I am happy with the de-rotation of my nose but the bridge wasn't augmented enough. I want a third surgery to raise the height of my dorsum. I am afraid that when my nose will be opened up again to insert the graft to raise my bridge height it could affect the septal extension graft. Is that a possible concern?

Answer: Dorsal augmentation revision Hello and thank you for your question. When appropriate technique is used during your revision, your SEG should not be affected. The technique I prefer for dorsal augmentation is diced cartilage wrapped in fascia. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Answer: Will septal extension graft be affected with a third rhinoplasty Revision rhinoplasty is one of the most difficult procedures to perform correctly in the entire field of cosmetic surgery, so it’s imperative to choose your surgeon based upon extensive experience. Your options are going to be a synthetic implant, rib graft, or even fillers placed in the nose. Since you have a SEG, you probably do not have any cartilage left over on the inside of your nose for grafting purposes. Ear cartilage does not work well in that location. Your nasal tip and SEG should not be affected by augmenting your bridge line.
