I'm not sure if you are referring to angle of jaw implants or the chin. A genioplasty procedure uses your own natural chin to be re-positioned in a more aesthetic position. For the angle of the mandible, custom designed implants based on your natural appearance using a CT scan generally provides the best results. There are numerous problems with attempting to use your own bone to enhance the angle of the jaw (resorption, asymmetry, donor site morbidity, fixation).

It is possible to use patient own bone from hip or scull by means of bone graft to augment jaws. This is major procedure that require additional donor site and scar(s), fixation with screws and has less predictable result due to partial bone resorption. This is by all means significantly more complex and expensive procedure in comparison to predictable jaw implants. Yes, one can use their own bone for a chin augmentation. The reason this is not done normally is that there would be another scar on one's body. The survival rate how long this implant is not predictable. A sliding genioplasty can be done where one's own chin can be moved forward but this would not be considered an implant. This is why most implants are made of silastic material. Besides the harvest issues, the amount of metal fixation needed to hold them in place and that the shape outcome would not be smooth, onlay bone grafts are associated with significant resorption. For bone grafts to stay and maintain volume they have to be functionally loaded. Adding more bone that is what is naturally there as an only graft has no functional purpose. Look up a surgery called a genioplasty. Essentially the bone is separated and slid down meaning that your augmentation is with your own bone. I'm not sure there is a great utility to it as its not like a silicone implant feels unnatural over time. If done correctly both techniques are equivalent. Hope that helps!
