I got a breast aug 7 months ago (455cc MP) and I'm disappointed. I expected them to be bigger and to have cleavage but I have a wide chest (I can fit my entire hand between my breasts). I am 22, 5'8, 165 ish lbs. I'm wanting to get bigger implants/fat grafted to give me cleavage but not sure? Would a different profile help? Are bigger implants going to make a difference for my cleavage? I'd rather not get lipo if theres a chance the fat won't stay.
Answer: Fat transfer to breasts Thank you for the question. A lot depends on your anatomy, size and position of your breasts in relationship to your chest. Fat transfer will help contour areas such as the medial poles of the breasts to enhance the cleavage. An in-person assessment and evaluation is needed to determine the best plan for you. I wish you the best of luck. Vasileios Vasilakis, MD. Double board-certified, fellowship-trained plastic surgeon, Houston. TX.

Answer: Mini breast lift Your breasts are widely separated. The technique you need originally was a breast lift to move your breasts more medially. At this time, I recommend a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.

Answer: Implants Bigger implants may not be able to give you more cleavage. If your breasts sit naturally far apart, they will remain that way. The base of your breasts cannot be moved. An examination if needed to see if fat grafting is an option. Fat is placed inside actual breast tissue and not in the pocket with the implants. If you have little tissue in the area you wish fat, then fat grafting will not work. I would suggest going back to your surgeon and discussing your concerns. Please do not chose an implant that is too large and heavy for your tissue and frame. Otherwise, you will just have big sagging breasts.

Answer: Revision surgery Dear Beachlover1999, breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery. Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size. While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals. Answer: Cleavage Dear Beach lover, A physical exam would do wonders in more accurately answering your questions. That being said, lack of cleavage is due to your chest anatomy with both a sharp curvature of your ribs which cause your implants to sit more laterally (combined with a wide sternum). If your breasts move closer with better cleavage when wearing a bra or pushing them, then your pocket is open. IF not, opening the medial pocket and possibly larger implants or a lower profile with wider base diameter may work. Best wishes. Dr. Taranow
