On the front and lateral views you look more droopy than really large. But since they feel too heavy, a lift with some degree of reduction is indicated. The amount to be removed would be done after careful consideration and consultation with your plastic surgeon.

Based off your brief summary of symptoms and photos - almost all of your issues could be related and stemming from the HRT. Now the question is, are you willing to stop the HRT or decrease dosage to reduce some of these symptoms? Or perhaps speak to your physician for alternatives that could be less symptom related. If you were able to reduce your weight, you would decrease your breast size as well as possibly some back and shoulder issues that are related to that aspect - which leads to all the other secondary symptoms on top of that.
As far as getting a breast reduction, yes you can potentially but review the different scars that are done online with breast reductions (anchor pattern, lollipop pattern) prior to seeking a few surgeons for opinions on your breast. It can be done but will it solve all those issues you mentioned? Likely not.

You do appear to be a breast lift (mastopexy) candidate with a small reduction. This will be nice to alleviate some weight and position nipple areolar complexes at the maximally projecting position on breasts.

Your situation is unique. While you can consider a reduction the breasts can grow again due to the HRT. From the photos it does appear you could consider a lift with a possible reduction. I encourage you to speak with your doctor about your situation to determine the way to best proceed.

Cup size is sometimes quite subjective. To me you are not a DD cup. A typical breast reduction usually removes a pound or more of breast tissue per side. A breast lift may be a more appropriate procedure.
