Plastic surgery scars require attention after surgery so that they remain cosmetically-appealing and barely noticeable. In our office, we use a protocol for scars that minimizes their activity in the 6-8 weeks after surgery. You have options depending on your goals. One would be to have a second reduction. This could improve the scars and fill the tissue defect. If you did not want to reduce your volume further, an option would be a scar revision and fat grafting to the indentation. Your concerns should be discussed with your surgeon who can provide more detailed information after conducting a physical examination.

Your breasts are still quite large and would benefit from a repeat reduction or scar revision. It may be possible to use the inframammary incisions to correct the abnormality and remove excess skin tissue or redo the entire breast reduction. The reduction technique I recommend is The Bellesoma Method. This will reduce and reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in pain relief without excessive reduction. Based on the submitted photos you have a couple options. It appears you would be a candidate for a secondary reduction. This would decrease the size of your breasts as well as correct the scarring and contour issues you mentioned.
