Does it appear that I have capsular contracture? If so, what level? (Photos)

There have been numerous attempts to grade capsular contracture over the years. The real question is how do you objectively describe hard. Most plastic surgeons rely on the Baker scale. Grade I is normal. Grade II is somewhat firmer than normal, but not much of an issue. Grade III is firm and can be somewhat distorted. Grade IV is firm, distorted and even painful. It's not a perfect description since there is obviously crossover, but this is the most common way to describe the condition. Based solely on your photos, You are either a grade III or IV, We seldom operate on a II, but a III and IV usually results in a need for surgery. Taking this one step further, it looks as though these are sub-glandular implants with a donut lift on the right side. If you are seeing the surgeon who performed the original surgery, they know what was performed. If it is a different surgeon, make sure that you have the records from you original surgery. It helps with the planning of any subsequent surgery. If you are currently nursing, most surgeons would wait until you stop before considering any surgical intervention.

Baker Grades: I-Breast is soft and appears natural in size and shape. II-the breast is a little firm, but appears normal.  III-Breast is firm and appears distorted.  IV-Breast is hard, appears distorted and there is pain.So going by that, you have a Grade IV.  In these cases the capsule should be taken out or at least completely collapsed closed to prevent any fluid accumulation (seroma).Hope that helps.

Probably III/IV contracture.  Once breasts have reached normal size after discontinuing breast feeding there may or may not be some improvement in this situation. If not then implant exchange and capsulectomy may be indicated, in addition to addressing what appears to be skin excess/laxity.  Best of Luck  Dr Harrell

Your right breast has a grade four capsular contraction by virtue of it being very hard and visibly distorted.
